Katilin & the Interactive Metronome

The use of the Interactive Metronome (IM), as a modality to enhance new learning patterns and motor coordination, is growing steadily in therapy settings across the country.  This diverse treatment tool can be adapted to cross the boundaries from pediatric to adult populations, who in many cases had given up hope of further gains. Portable and easily set up to suit a patient’s individual treatment plan, clinicians are exploring new pathways to impact an individual’s learning style.

Over the course of treatment patients learn to connect to an auditory computer generated stimuli requiring a specific motor response.  Each response is measured by the computer program in milliseconds, to detect response speed and accuracy of patients using diverse motor patterning.  Over time patients adjust to increased motor challenge combined with increased repetition of motor response, which in turn leads to the laying down of new motor pathways and fluidity in motor patterning.  Both auditory and visual systems can be isolated to assist in attention to task.  Gross and fine motor coordination can be targeted for a goal-specific response.

The development of the home product, IM-HOME, has expanded treatment to family settings, allowing increased frequency of use and the comfort of home surroundings.

Case Study- Kaitlin
Kaitlin could never have been described as a quiet and unassuming child.  From an early age she learned to take control of her environment and was often found climbing cabinetry and stairwells in her house.  However, this outward appearance of self-confidence and often open defiance, were dichotomous to true feelings of decreased self-worth, inferiority and overall lowered self esteem.  At the age of 6 years of age it emerged that this front could no longer suffice as a protection strategy.

Kaitlin’s teachers were aware of the challenges that she presented with comprehension and processing and recommended further testing be performed to come up with a diagnosis.  As a student of a private school, OT services were not available as a referral source.  Through private OT consultation an initial plan and prioritization of issues was set in place as follows:

  • Integration of visual motor skills to whole  task activities

  • Increased length of focused attention to non-preferred/challenging activities

  • Proximal stability/core strength during static tasks

  • Complete standardized testing in clinic and school based settings to establish possibility of diagnosis related to learning disability

  • Sustaining attention and compliance to IM-HOME

  • Vocal regulation and articulation

  • Recognition of body part identification during motoric activity

  • Relaxed body posture and engaged facial exchanges during communication

As a case in progress, initial concerns were voiced by her parents, teachers and close friends.  Kaitlin commenced an IM program designed to aid attention and entice engagement.  Activities were set between each IM exercise in a “Funblast” program format.  This empowered Kaitlin to participate as the choices remained up to her.

Motivational activities were established with Kaitlin’s participation as follows:

Funblast Program

  • Juggling Sticks

  • Balance activities on taped line

  • Tray ball toss

  • Visual scanning of preferred picture during marching/stepping activities

  • Eating a snack while using IM foot trigger

  • Bouncing on trampoline

  • Balancing on Bosu

IM-HOME repetitions were increased weekly from 4 sets of 54 to 8 sets of 54 over a 3 week course.

Following an initial 3 week program with frequency at 3 times per week, Kaitlin presented with the following gains:

  • Improved articulation

  • Decreased mumbling during challenging activities

  • Increased confidence for novel motor play (use of “Heelies” - her own personal best!)

  • Decreased sensory defensiveness to noxious stimuli

  • Teachers report improved postural control during class (leading to observation of visual tracking difficulties in isolation)

  • Award “Star of the week” for good behavior

  • Successfully and competently “posting” during horseback riding lessons

  • Increased participation in homework activities

As Kaitlin has shown such promising results in such a short period of time, her family plans to continue consultation with OT and develop further IM-HOME activities for a further period of 3 to 6 months.  Goals will continue to focus on Kaitlin’s overall self esteem and ability to embrace new challenges without the need for hyper-defensiveness or anxiety.  As the weeks pass we are discovering a whole new Kaitlin emerging, as she uses the IM.

United Way Grant enables IM to be adopted by the Boys Club of Sioux City

Women United grant promotes brain development

United Way's Women United has announced that $40,000 will be awarded to the "Brain Zone," a ground-breaking project implemented through the Boys Club, Mary Treglia Community House and Girls Inc.

The three United Way agencies will collaborate to provide skills and activities to help children develop their brains. Brain Zone activities have been proven to help improve children's thought process.

Roger Friessen, executive director of the Boys' Club of Sioux City, has promoted Brain Zone as a way to help children with learning disabilities.

"Thanks to the generous contribution from Women United, it will be possible for us to test and identify children with learning disabilities, and help them, improve information processing, to overcome behavioral and learning difficulties," Friessen said.

Brain Zone consists of three program areas: Interactive Metronome, BrainBuilder and Bal-A-Vis-X, a series of Balance/Auditory/Vision Exercises that require full-body coordination and focused attention to integrate both sides of the brain in a students' thought process.

Friessen added, "In the past, many children with brain processing related learning disabilities were never identified. These children were left to struggle through life without relief from the symptoms they exhibited. They found themselves unable to cope with daily life because they were unaware they even had a physiological problem; therefore they never sought help or treatment. They struggled with anger management, poor work performance and attendance and self-worth issues that affect virtually every aspect of their lives."

The "Brain Zone" project will begin working with children at the Boys' Club, Mary Treglia Community House and Girls Inc., as well as Lowell Elementary School. Goals include expanding the program to other schools in Siouxland.

"We are very excited to provide the resources necessary to grow 'Brain Zone' activities within our community,", stated Charese Yanney, co-chair along with Karleen Waller of United Way's Women United. "This project truly demonstrates United Way and Women United's commitment to addressing the root cause of issues that affect Siouxland youth. Brain Zone will achieve some significant change for the better in Siouxland."

Waller stated, "United Way and Women United have made it possible for women to pool their collective resources to invest in programs we feel passionate about and that make a visible difference. In fact Women United has identified its mission as: providing financial support to organizations or groups that demonstrate the ability to enhance the quality of life for our Siouxland youth."

Waller added that the Yanney family, Charese, Do and Jim),was instrumental in bringing the Women United concept to Siouxland. "They saw how this concept empowered women and raised additional funds in a community that could be leveraged to make an impact."

This is the second project for Women United. In 2006, a grant was awarded to the Council on Sexual Assault and Domestic Violence to launch "New Beginnings," a program that helps break the cycle of violence and domestic abuse by helping families as they transition from living in an unsafe environment to a safe, self-sufficient life.

What are the hosting requirements?

  • The facility must be ADA compliant

  • A room available that is large enough to accommodate 20 to 40 people set-up classroom style

  • The room must be available from 6am to 6pm

  • You will need to provide an LCD projector and an 8 by 8 screen for the duration of the course

  • As the host, you are required to set the room up (please see preferred room layout attached)

  • One electrical outlet per every four seats is preferred

  • Provide coffee, tea, water and bagels/muffins in the morning and cookies and sodas in the afternoon.

How long is the course?
The course is just one day. It consists of eight hours of teaching with a one hour lunch break. Registration starts at 7:15 am and teaching begins at 7:45 am. The course ends at 5:15 pm.

Will CEUs be offered?
Yes. OTs/COTAs, SLPs/SLPAs, & ATCs can earn .8 CEUs. *Please note we must have board issued license numbers.

Will I be Interactive Metronome Certified at the end?
As long as you pass the exam with an 80% or above and complete the evaluation you will be Interactive Metronome Certified and considered an IM Provider.

  • Perform & interpret objective IM assessments competently;
  • Create & execute individualized IM treatment utilizing six phases of IM treatment;
  • Produce and interpret reports;
  • Select sensitive pre and post assessment tools to measure effectiveness of IM &  generalization of gains to settings outside of the clinic;
  • Identify IM exercises can and will be modified to meet individual patient needs;
  • Develop individualized goals for IM treatment that are both functional and measurable.

Specific Learning Outcomes for SLPs:

  • SLPs will demonstrate evidence-based practice through knowledge regarding available research and supporting literature for use of Interactive Metronome® (IM) in the remediation of speech/language and cognitive/linguistic disorders;
  • SLPs will demonstrate appropriate clinical application of IM in pediatric, adolescent, and adult/geriatric patient populations for individualized remediation of developmental and acquired neurogenic speech-language & cognitive-linguistic disorders;
  • SLPs will demonstrate objective, measurable & functional goal-writing in order to gauge meaningful IM treatment outcomes;
  • SLPs will select appropriate speech/language and cognitive/linguistic assessment batteries to objectively measure changes pre- and post-IM treatment;
  • SLPs will select appropriate diagnosis and CPT codes to maximize reimbursement for IM assessment & treatment.
